How did you start compounding? What led you to PCCA?

Our pharmacy was founded in 1964. As a young boy, I watched my father fill prescriptions, and every script was a compound. Times changed and we got away from compounding. Then, in 1997, we joined PCCA because we wanted to return to our roots and have a more meaningful impact on patient outcomes.

How do you “Keep Things Fun” at your pharmacy?

PCCA’s Principles align with HB Pharmacy’s principles, so it’s hard to choose just one, but I like this one a lot. We need to take our work seriously, but without laughing or having fun, people often become robotic and are not the best version of themselves. We keep the atmosphere fun and light and laugh often, but, at the same time, we are diligent in our duty.

What are your favorite PCCA Principles, and how do you live these at your pharmacy?

Bring Your Best Self Every Day

As an independent pharmacy trusted by our local community, we take pride in helping patients in need. We especially feel this pride with our patients who receive compounded medication, as their needs are not met with the traditional medications provided by traditional pharmacies. Therefore, many of these patients rely on compounding pharmacies like us. Knowing that these patients go through a lot to have their prescription filled, we always try to go the extra mile to meet their compounding needs with a quick turnaround time for them to start their therapy ASAP.

This Principle can be taken more than one way. First, HB Pharmacy is a family. Whether you are a pharmacist, technician, clerk or delivery driver, we always listen to what our team members have to say and hold every suggestion valuable. There is a mutual respect and recognition that we are a team that works together every day to ensure we provide the best possible outcomes. One example of phenomenal collaboration goes back to October 6, 2022. Dr. Pamela Smith was here to speak, and our team performed in harmony, like a well-oiled machine. It was a real sight to see, and I could not be prouder to work with all of them. The result of their efforts was an amazing night where our guests left with valuable information and enthusiasm to affect changes in their respective practices.

When was the last time a patient thanked you and why?

Over the years, we have been blessed to have helped many patients who were truly in need. One case that comes to mind though concerns a patient who was battling cancer and was on heavy doses of chemotherapy. The side effects became debilitating. The one thing he wished he could do was eat or drink without pain. Working with the prescriber, we created an oral neuropathic pain gel for him containing lidocaine, ketamine, ketoprofen and gabapentin. The real key to the compound was the base. We used MucoLoxTM, which is an amazingly effective and extremely unique base. The way MucoLox adheres to the oral mucosa is unlike any product I have ever compounded with, and the relief our compound gave our patient was dramatic. I have never encountered a more appreciative patient.

What is your favorite PCCA education program or event and why?

Speakers Bureau. We have held four Speakers Bureau events to date. We had the nationally known Anti-Aging and Functional Medicine Specialist, Dr. Pamela Smith, come to our pharmacy to educate our local prescribers on hormone replacement therapy.

What is your favorite PCCA base and why?

Although PCCA has a plethora of unique bases, including Plasticized™ Base, PermE8®, WO6®, XemaTop™, etc., I would have to say VersaBase® Cream! It is such a versatile, yet easy base to manipulate. Our "default" base is always VersaBase for most hormone therapy patients. Our patients are very selective in the products they use, especially the added ingredients they may contain. It’s hypoallergenic and pH balanced! We get rave reviews from patients on this base. It’s like the “classic” of innovative bases. 

What advice would you give someone who is new to pharmacy compounding? 

Maximize your resources. PCCA offers a variety of resources to use as a reference. When you are new to pharmacy compounding, these resources can help tremendously — from a simple formula search to more individualized formulary support, help from PCCA Clinical Services and marketing resources to marketing education. Make sure to utilize what you can that comes with the membership. PCCA is a high-quality chemical supplier, but offers so much more.

This Profile in Personalized Medicine highlights PCCA’s 2022 Canadian Pharmacist of the Year, Michelle Gray, BScPharm, RPh, C4 HRT Specialist, Therapeutic Nutrition Lifestyle Coach and owner of Gray’s Compounding Pharmacy in Kimberley, British Columbia. She has been a proud PCCA member since 2013.

How did your pharmacy compounding story begin?

I was hired by a small independent pharmacy in 2002 to start the compounding practice for the owner. The owner and I went to Houston for C3 training. This along with the daily PCCA clinical support gave me the confidence to go out into the medical community to educate physicians about what compounding is and how I could help them with their challenges. They would then write prescriptions when appropriate, and I would go back, make the preparations and counsel the patients.

What was the moment you realized that joining PCCA was going to make a difference for your pharmacy?

The independent pharmacy I worked for was sold to the Shoppers Drug Mart chain in 2006. Along with the sale, we lost our PCCA membership. So, having to use a competitor, I phoned them for a formulation consultation to help me solve a problem. I was shocked when they said it would be two weeks for them to come up with a formulation. I told them the patient doesn't have two weeks, they have two hours! So I was on my own. Using what I had previously learned at PCCA, I came up with a formula for a very bitter suspension. In 2013, when I opened my own compounding pharmacy, there was no question in my mind about the value that PCCA would bring to my practice, myself, the clinicians I collaborate with and, most importantly, the patients I serve. Being a PCCA member also means we have access to the highest quality chemicals, clinical services and education.

How have you tried to make Gray’s Compounding Pharmacy different from other pharmacies and who are the people who make it work so well?

We try to look at our patients’ whole health, with an approach that considers the various systems and how they affect one another and overlay that with a functional medicine approach. Sometimes a fresh set of eyes can bring new solutions. We collaborate with healthcare providers with the patient as the main focal point. Peter Wu, our pharmacist, is not just filling prescriptions, he’s actively assessing what’s best for the health of his patients by analyzing lab work, performing high-level clinical reviews and pain consultations.

Heidi Radcliffe and Kayla Morrison are our registered pharmacy technicians. They are integral in so many aspects of our business. We’ve sent them away for specific training in drug compounding, they perform risk assessments and ensure we’re always following the highest regulations of the National Association of Pharmacy Regulatory Authorities.

Our pharmacy assistants are Corina Howorko and Felicia Green. They are so efficient and able to quickly shift gears to take care of all pharmacy operations. That frees up Peter and I to spend more time with patients. It has changed the focus at Gray’s Pharmacy to a much more caring, in-depth, whole health service.

And of course, there is Andy Gray, my husband. We call him the fixer. He is trained in compounding and fills in everywhere – not to mention he’s coordinating taking care of our three children, our dog and even the exchange student in our home!

As PCCA Canada’s 2022 Compounding Pharmacist of the Year, what is something you would tell people who want to follow in your footsteps?

I am passionate about helping people and using every tool I have developed. I am proud that my ongoing efforts have been recognized and hope it inspires others to follow in my footsteps. I would tell newer compounding pharmacists to listen to all of their patients closely. Ask about their families. Inevitably, someone has an issue that is an opportunity for you to help with. I was surprised when I got a call from a community 600 km (373 miles) away. It was a patient whom I had helped years before with a burn when he was on holiday near my pharmacy. He was now calling for assistance with his sister in the next province who had a diabetic ulcer. You never know when you will be needed, and being present for every patient encounter will earn you a reputation as a "healer.”

We recognize your goal of including pharmacists in the “triad” of care (patient, physician and pharmacist). Is that what inspired you to get into compounding or was it something else?

Initially, care for patients on a more personal and customized level created my motivation to begin our compounding journey. As the years have gone by, developing and fostering deep and meaningful relationships with prescribers has become a larger part of our practice. It is powerful and rewarding as a pharmacist to have patients and physicians rely on you and achieve better outcomes through the high level of education and quality compounding we provide. These strong bonds create a solid backbone for the “triad” in our community.

You opened your pharmacy in 2002—the same year you became a PCCA member! Were you aware of PCCA before deciding to offer compounding services? Who or what made you aware of PCCA?

Before opening in 2002, I created a business plan and leaned heavily on my friend and mentor, Fred Emmite, RPh. As a personal friendof PCCA CEO Dave Sparks, Fred suggested I look into this compounding company in Houston, as it may hold the keys to success in my pharmacy future. Little did I know at the time he was so right. The knowledge and expertise that PCCA provided me and my staff from the day we opened set the stage for our personalized medicine journey. I cannot imagine our pharmacy operations and my life without PCCA.

We’re certain you’ve had many memorable patients. Is there one, however, who really stands out?

Several years back, we started compounding a combination of a low-dose steroid and low-dose antibiotic for eczema. The doctor who developed the theory behind its use has a large worldwide following and quite often we have patients reach out to us about this regimen for their infants and children who suffer horribly from eczema. One Friday afternoon, a father of a one-year-old infant called and told us the story of his child and how the eczema was so bad it prevented the child from sleeping, eating and even wearing clothes. Despite receiving the order for the compounded medication after 4:00 p.m., he begged us to get the medicine ready for his child before the weekend. Hearing the agony and distress in the father’s voice, we agreed to stay over and get the compound ready. On the way to pick up the prescription, he called and asked if my staff and I liked crawfish. We said that was a no-brainer! When he showed up to pick up the prescription, he brought 10 lbs. of boiled crawfish for us. After he left and we closed the pharmacy for the evening, my staff and I sat out on the tailgate of my truck and enjoyed the crawfish. We later found out from the father the compound cleared up the infant’s eczema and improved the entire family’s quality of life. A win-win for everyone.

Your pharmacy celebrated a milestone anniversary in 2022. What or who do you attribute your pharmacy’s longevity and success?

Our pharmacy has always operated under the motto: If you take care of patients, as a side effect, you can make a living. That simple principle has been the driving force for our operations. Another secret to our success is having a caring staff that comes to work each day treating the pharmacy as if they are the owner. Creating a culture that fosters an outward mindset and hiring employees that are empathic, compassionate and conscientious has made our growth and success possible.

You’ve received many awards—including PCCA’s 2022 Pharmacist of the Year (congratulations!). Given your professional success and recognition, what advice would you offer those who are either interested in or starting to compound?

Of all the accomplishments and rewards I have received in my career, being awarded Pharmacist of the Year highlights the dedication I (and my staff) have committed to quality compounds and improving the lives of our patients. No other area of pharmacy can truly change, heal and serve every patient need like that of a compounder. The current state of traditional pharmacy has made the avenue of starting or growing compounding services more important than ever. Being a compounder requires a tremendous level of commitment and dedication. Lifelong learning is a must and creating a culture for your staff to grow professionally will develop a positive and nurturing workplace. Today’s patients are looking for wellness and healing. Whether you call it functional, integrative or personalized medicine, providing a healthcare destination with unique solutions provides for a career that is professionally rewarding like no other.

Is there anything else you’d like to share?

The future of our profession as compounders will only be possible with advocacy. With PCCA’s guidance and support, we must stay engaged on a local, state and federal level to ensure our patients do not lose access to customized medicine.

How did you get into pharmacy compounding?

I worked at People’s Custom Rx for three years in pharmacy school and came to love compounding. After several years, I got to come back, and eventually bought the pharmacy five years ago. The previous owner was a PCCA member, and I knew I had to be a member to help the pharmacy business continue.

What is your favorite thing about your job?

Getting asked, “Can you...?” and getting to say, “Yes, we can!”

When was the last time a patient thanked you?

Just the other day a patient needed some amlodipine tablets split in half. Even though we are a compounding-only pharmacy, I had what she needed and split them for her. She was very grateful. Sometimes it is the little things that make the biggest difference.

What is your most satisfying patient experience?

Working with a veterinarian at the Memphis Zoo to help a stingray with a wound. We used a Polyox® bandage formulation to help it heal.

What is your biggest "WOW" moment as a PCCA member?

The PCCA community of compounders. We try to help each other succeed while working through all of the regulations.

What is your favorite PCCA Base?

ZoSil®. It’s pet friendly and helpful to our veterinary patients.

What is your favorite PCCA program?

The Veterinary Symposiums. Of course, because of what we learn, but also because we get to go to a zoo!

What does “Compounding: It’s Personal” means to you?

The goal is to help an individual where they are and with what they need.

What is a piece of advice you would give to new compounders?

Get informed on regulations. Then, get involved and stay involved with professional associations.

What led you into the career of a pharmacy technician?

I wanted a stable and interesting field. I decided to go to school to become a technician and once I was in the program, I realized I thoroughly enjoyed it. After I graduated, I became nationally certified and wanted to make sure I learned and absorbed all vital information regarding pharmacy once I started working at Patio Drugs Pharmacy.

When did you learn about compounding?

When I was in school, I had the opportunity to take an IV/compounding class and I immediately knew this was the aspect of pharmacy I wanted to specialize in. I moved to Louisiana where Patio Drugs was the leading compounding pharmacy in the area (and still is). Once I was aware there was an opening, I applied and was blessed with the opportunity to learn and work alongside the best. I started compounding in February 2010; I have been compounding for 12 years and in this area of pharmacy, I learn something new every day.

What do you find most fulfilling about compounding?

The most gratifying facet of compounding is being able to assist our patients and doctors in providing treatments that are personalized to their specific need or requirement.

What are some of your favorite PCCA formulas?

Nasal suspensions and solutions are big for us.

A favorite base of mine is NataTroche™. It’s a pleasure to compound with since it is an "all-in-one" base that makes creating the troche way more efficient. And I cannot forget to mention the PCCA Rectal Rocket, as it is the epitome of pharmaceutical elegance.

What are some compounding success stories of yours?

There are many achievements at Patio Drugs Pharmacy that have had meaningful impacts. We take great pride in being a community pharmacy and make sure we best serve our patrons in any way possible.

One time in particular stands out in my mind. We had a patient who was an organ transplant recipient and could not take the necessary lifesaving commercially available medication. Our compounding team researched and found a formulation to make the important medication into a suspension so they could continue their treatment uninterrupted and alleviate any concerns the patient or doctors had.

Patio Drugs Pharmacy also serves animals. We are able to make custom dosage forms to ease the administration of medication to our beloved pets. I have two cats, Loki and Tootsie, and I’ve learned how difficult it is to give them medications.

We once received a call from a dog owner, asking if there was any way we could help with medication administration. The dog would not take liquid medication and they’d already tried capsules with little to no success. Working with the veterinarian, our compounding team came up with a resolution. The dosage form was reformulated into a gel using a penetration-enhancing base that facilitated the administration of the medication. Happy pet, happy life.

What advice would you give to a new compounder?

The utmost advice I can convey is to learn the fundamentals. Learn the math, the laws, the correct sterile/nonsterile compounding techniques, policies and procedures. Hold yourself to the absolute highest standard; error is not an option. Double-, even triple-check all formulations. Compounding pharmacies address and serve patients in need of solutions when others do not have the answers.

When was the last time a patient thanked you?

Recently, my patient appreciated a suggestion I made to his doctor for his topical pain cream, which really improved his quality of life.

How did you come into pharmacy compounding?

I credit my mentor, Mr. Mohamed Walji, a senior member of PCCA, as the reason for why I am compounding today.

What is your favorite thing about your job?

Challenges! Whether it involves sourcing, creating or pricing, I find that it’s fun learning new things and being appreciated for what you do. Above all, I value that feeling of achievement. Work feels like home and is something I look forward to every day.

What advice would you give to new pharmacy compounders?

Always talk to patients. You need to earn the confidence and trust of the patient for them to believe in what you are compounding.

What is one of your most satisfying patient experiences?

My toughest patient challenges involve the administration of rectal rocket compounds. With this dosage form I feel that we consistently see positive outcomes; however, I always make sure to communicate up front with prescribers to ensure patients are mentally prepared for and comfortable with the treatment.

What has been your biggest "WOW" moment as a PCCA member?

Recently, my pharmacy provided an acne formula to a local health care provider. The practice documented the formula’s outcome for their patients with melasma and acne by taking before and after pictures. They then shared the pictures with me, which is an excellent marketing tool. The results achieved in just four weeks were a total “WOW” moment!

What is your favorite PCCA base?

Lipoderm®. I just love the results that patients report when using formulations we create with this base.

What is your favorite PCCA educational program?

The C4 Dermatology online course. I have learned so many new things through this program. Believe it or not, people care for looks more than health!

Did you always want to work in the field of pharmacy? Why? If not, what led you to pursue it as a career?

Not really. I grew up wanting to be a football player and, of course, as a kid dreamed of playing professionally. Tackling and throwing touchdowns felt natural for me and kept me excited. I feel like now I have a different calling. Now I tackle formulas that need fixing and ways to improve workflow in our pharmacy. For me, being a part of a team that helps patients get better with our custom compounded medications is like completing a touchdown. Not sure what led me toward this career path, but I find it very enjoyable. I also feel like we’re making a difference in patients' lives.

When did you learn about compounding? What brought you into the profession?

I learned compounding about 12 years ago at a compounding pharmacy in Las Vegas. When I started compounding, I learned that there are so many different formulas for all sorts of conditions, and I’d learn something new every day. It is satisfying to know that we can customize something from a pure powder to any dosage form that doctors prescribe or to fit a patient’s need. It’s truly a blessing that we can see patients’ lives getting better from the quality of work we do.

What do you find most fulfilling about compounding?

I get to train and share my knowledge with other technicians, but also get to learn and share ideas with them, too. Watching technicians grow into great, skilled and confident compounders is very fulfilling.

What are some of your favorite PCCA formulas? What makes them effective?

One of my favorite and most prescribed formulas is the troches compounded with PCCA NataTroche® base. We receive great patient feedback on these troches due to their effectiveness, and also because they contain a natural sweetener and flavor.

Please share a compounding success story from your pharmacy—one where a person or an animal was significantly impacted by compounding.

There are so many successful stories to share but the biggest success at Koshland Pharm is our entire team, and the dedication that we all give to our patients. It starts with our processing team taking the prescription orders and giving great customer service. Our pharmacists then do a lot of research and devote their time to consultations. The tech team puts in quality work, providing attention to detail with every compound. And our shipping crew/lab assistants make sure every medication is sent out on time and to the right place. Also, our marketing team builds the strong relationships with our doctors and practitioners, and continue to grow the company. It’s truly a team effort!

What advice would you give to a new compounder?

Every day is a learning experience. Always be open and ready to learn new things. But the best advice I usually give is to always have patience and take lots of notes.

When was the last time a patient thanked you?

Just recently, when I met a patient after hours to fill a fertility compound because she did not receive it. We were able to provide her with progesterone suppositories for pregnancy support.

How did you get into pharmacy compounding?

I was exposed to compounding early on in pharmacy school. John Carson did a presentation on compounding and independent pharmacy ownership at an NCPA meeting at the College of Pharmacy I attended. His presentation and enthusiasm about pharmacy compounding left a long-standing impression on me. After that, PCCA’s reputation led me to them.

What is your favorite thing about your job as a compounder?

My favorite thing about being a compounder is our ability to make an impact on patients’ lives that would otherwise not have been possible through conventional therapies.

What advice would you give to those new to pharmacy compounding?

Evaluate your process often. Strive for consistency and excellence before cost. Never forget about your obligations to your patients and the pharmacy profession.

What is your most satisfying patient experience?

We once had a 180-pound, 12-year-old patient who was fighting his parents when it was time for him to take his medications. Using Lipoderm^® to deliver the medications through his skin was life changing for his family. His parents told me, “You have helped bring our son back to us.”

What is one PCCA base you rely on regularly?

Lipoderm. It is extremely easy to work with, and its ability to deliver medications through the skin has proven it to be an invaluable asset to my patients.

What words do you live by?

Always do the right thing, even when nobody is watching.

Did you always want to work in the field of pharmacy?

My grandfather owned a pharmacy in Coatesville, Pennsylvania, and ever since I can remember, I loved going to his pharmacy after school. I would watch him fill the prescriptions. He would explain to me what he was doing, and I would think, maybe I can do this one day. I’ve always looked up to my grandfather. So, after many conversations with him and my father, I knew owning my own pharmacy was not only a dream of mine, but it was truly in my blood.

When did you learn about compounding? Why did you start compounding?

During my clinical rotations at USP, I met Mark Pimley, who was the owner of The Compounding Pharmacist at the time. He introduced me to the world of compounding. I found it very intriguing because I have always been a hands-on type of person. I always knew I wanted to do some compounding as a pharmacist, but it wasn’t until I called Mark four years ago for some advice that he offered me a position at this pharmacy, which ultimately led to me buying his business.

What do you find most fulfilling about compounding?

I love that I feel like I truly help people. There are many patients who require specialized medication, whether it be for certain allergies or untraditional strengths of a certain medication. These patients need my help, and it’s always nice to hear when they call me back and tell me “you’ve changed my life,” or how they've “never felt better.” Those calls make all that hard work worth it.

What is your favorite PCCA Formula?

My favorite formula is the rectal rocket suppository with hydrocortisone and lidocaine. The rectal rocket has been a great marketing tool when I go to gastroenterologist offices and offer this treatment option as a potential alternative for hemorrhoids.

Could you please share a compounding success story (or two) from your pharmacy?

I recently compounded a diaper cream for a child with special needs. The girl has bowel incontinence, and her skin was extremely raw. Her pediatrician called me for advice, and I recommended a cholestyramine, nystatin and mupirocin compound. The doctor prescribed it, and it cleared her rash up in one week. Her mom and the pediatrician were so appreciative and thankful.

What advice would you give to a new compounder?

Ask a lot of questions. I call PCCA Clinical Services all the time to ask questions to ensure that I make the best compounded medication. I would also encourage new owners to join the Concierge Compounding group for pharmacy owners. This has helped me tremendously since most owners experience similar issues.

How did you start compounding? What led you to PCCA?

While I was going to pharmacy school in 1983, I worked at a compounding pharmacy. They were PCCA members.

What was one of your toughest patient problems? How did you solve it?

I had a cross-country truck driver who was in a tough predicament. He had already tried to treat a common condition with suppositories, creams, etc. We talked to his doctor and then decided to purchase the Rectal Rocket mold from PCCA and compounded lidocaine, hydrocortisone and nifedipine Rectal Rockets for him, which the doctor had prescribed. That was exactly what he needed, and he ended up calling to tell me he was back to cross-country trips again.

What has been your most satisfying patient experience? What was their health challenge, and how did you assist them?

I was working at a pharmacy in Wimberley in the early ‘90s, and we had a family that was fostering a very sick premature baby. She was in hospice care and had a gastronomy tube. Her meds kept clogging her tube, so we consulted her doctor and started compounding medications so that it wouldn’t get clogged anymore. That family ended up adopting that baby girl, and she just celebrated her 30^th birthday a couple of months ago!

What was the moment you realized that joining PCCA was going to make a difference for your pharmacy?

Every pharmacy I had worked in prior to owning my own was a PCCA member, so when I did finally purchase my own pharmacy, I knew I had to become a member.

Do you have a favorite PCCA base?

I really like Lipoderm^® because it’s so versatile.

Do you have a favorite PCCA educational event?

PCCA’s International Seminar is my favorite. Everyone is there, and you’re able to get caught up on all the latest news in pharmacy compounding.

Can you recall a recent time a patient thanked you?

We really have awesome patients. They are always thankful and appreciative. Just this morning, we had the owner of a local palliative care company come in and tell us how grateful they were to us for always getting their patients taken care of so promptly. That kind of thing means so much to me.

What advice would you give to new pharmacy compounders?

Make sure to use PCCA because their vast knowledge of compounding is unmatched. It’s also important to develop relationships with the practitioners and veterinarians in your area.

How did you start compounding, and what led you to PCCA?

I was hired as a pharmacist right out of pharmacy school to work in the sterile compounding lab at Utah Family Pharmacy, which is owned by Cliff Holt. He was already a PCCA member at the time. I loved it, though, and loved being able to use more of my chemistry knowledge from school. With time, I began to supervise the nonsterile operations as well, and I worked up to the position of being our hybrid pharmacy organization's only compounding pharmacist. I was fortunate to learn so much from such a knowledgeable, patient mentor as Cliff.

What was one of your toughest patient problems, and how did you solve it?

As the only community pharmacy in southern Utah that provides sterile compounding services, we're often called on to prepare medications for unique cases. This has required creativity in formulations, while also making sure we stay within the guidelines of the various USP chapters and published scientific data. Some of the most difficult cases we've experienced have been patients with severe, vision-threatening eye infections. We've been able to work with local ophthalmologists to prepare anti-infective ocular injections as well as topical medications for follow-up treatment. It's satisfying for me to know that we've made a difference in the vision and lives of patients in our community.

What has been your most satisfying patient experience? What was their health challenge, and how did you assist them?

This is a tough question. Over the course of my short career, I've had so many opportunities to help patients with unique challenges. It's hard to pinpoint one experience, but one of my favorite patient populations to help are pediatric patients. We've had pediatric patients with organ transplants, eye problems, skin conditions, ear infections, almost any condition imaginable. I've been able to use my knowledge and resources to help these patients. Knowing that I've made a difference in not only the health care of a child, but in their quality of life when their family felt like they might not have any other options, is one of the greatest experiences in my career.

What is your favorite PCCA base?

I have loved working with Ellage^®. That has been a game-changer for us. It's great to have a vaginal base that is anhydrous, but also has great mucosal adhesion. The patients we've used it for so far have loved it.

What was the moment you realized that joining PCCA was going to make a difference for your pharmacy?

I came into a pharmacy that was already a member of PCCA. I have seen how much PCCA has helped us succeed in caring for our patients, though. Through educational conferences, Clinical Services, technical support, the considerate attention of sales representatives, and the quality of products and bases, our membership in PCCA has made a tremendous difference in the success of our compounding lab, and our pharmacy in general.

What is your favorite PCCA educational event?

I always enjoy International Seminar. I'm quiet by nature and don't mingle as much as I'd like to, but I do enjoy rubbing shoulders with other incredibly wise and skilled pharmacists and technicians, hearing what others are working on and developing, and bringing home concepts from presentations that I can use to better help and serve my community.

What advice do you have for new compounders?

Use the knowledge and experiences of other compounders, and establish close relationships with the physicians. And pharmacists, never underestimate the value of having strong technicians who care as much about taking care of patients as you do.

How did you start compounding? What led you to PCCA?

I had previously worked at a compounding pharmacy in my area. This pharmacy was sold and stopped compounding. In the meantime, I had purchased my own pharmacy and recognized the opportunity. I was familiar with PCCA from my previous workplace and knew I needed a full solution (support, supply and training) to get up and going.

What was one of your toughest patient problems? How did you solve it?

My toughest patient problem was a 10-year-old boy who needed treatment with fluoxetine for anxiety associated with autism spectrum disorder. A 5 mg/mL liquid formulation with a 1 mL dose was prescribed since the boy was unable to swallow capsules. Fluoxetine HCl is one of the least palatable medicines I can think of, so in our formulation, we deployed all of our best ideas to improve the taste — sweeteners; SuspendIt^®; marshmallow flavor; and the secret weapon, Bitter Stop^™. The patient still refused to take the medicine. We tried banana flavor. Sadly, that didn’t do the trick, either. We tried a flavor combo of marshmallow and strawberry. It was still not a winner. We had success at last with a fluoxetine 1 mg/mL 5 mL dose because the concentration was lower and we were able to hide the taste more.

What has been your most satisfying patient experience? What was their health challenge? How did you assist them?

My most satisfying patient experience has been helping a young baby suffering with gastroesophageal reflux disease (GERD). We made an omeprazole suspension for the baby according to the doctor’s prescription, but the child still had symptoms. At this time, I had a conversation with PCCA about the formula, and the suggestion was made to try a maximum dose volume of 2 mL.

At this point the baby was only six weeks old, and Mum was yet to sleep! She was also ready to give up on the omeprazole. After a chat about it, Mum was willing to try this idea, and we got the approval from the prescriber to change the formulation. For the first time since the baby was born, everyone in the house was asleep at the same time. The next day, I received an emotional phone call — Mum was extremely grateful.

What is your favorite PCCA base, and why?

WO6^® Anhydrous Topical Gel. This base feels more like an expensive cosmetic than a vehicle for medical treatment. It also offers extended beyond-use dates for compounds, which patients, prescribers and pharmacists love!

What is your favorite PCCA educational event, and why?

PCCA Australia’s Annual Conference, especially the one from June 2017. The best presentation was by PCCA member Jeff Carson from Oakdell Pharmacy in San Antonio, Texas. His “What’s in the box?” discussion was about making points of difference for your pharmacy practice.

What is a recent patient success story?

Yesterday! The patient had visited four other pharmacies to see if they could make her compounded medication, and we were able to have it ready on the same day.

What advice do you have for new compounders?

Call PCCA every time!

What was the moment you realized that joining PCCA was going to make a difference for your pharmacy?

I realized the value of PCCA at the very beginning. My pharmacy was a compounding startup, and there was so much to learn that I had not had to consider in my previous practice in a regular pharmacy — how to package compounds, when compounds should not be compounded, pitfalls to avoid and the rest. PCCA has always been there to offer me support.

How did you start compounding? What led you to PCCA?

My experience in compounding started very early in my career. As a student in college, I would compound total parenteral nutrition (TPN) preparations and antibiotics for home-care patients. Shortly after graduation and as a new pharmacist in 1992, I compounded medications for children with pediatric HIV during a time when there were very few available pediatric dosage formulations. Providers asked for pharmacy collaboration to help with supporting patient adherence. So I would make gummies and lollipops and suspensions flavored and tailored to the needs of each patient. Basically, I would collaborate with the family and provider to help these children take their HIV medications. I then started doing some pediatric AIDS Clinical Trial Group studies with the National Institutes of Health and a local hospital, and that gave me the ability to come up with other formulations for the kids.

As part of my commitment to learn more about compounding, I attended a PCCA training in Houston during the early 90s after my father had met with PCCA’s Bill Letendre. I have been growing my compounding practice ever since.

What was one of your toughest patient problems? How did you solve it?

Solving problems is what I do, for patients and prescribers alike. One of the most challenging patient problems I can think of involved a patient with a diagnosis of shingles. This patient had numerous lesions covering a large surface area from the back of their head through their neck and extending down to their shoulder. They were on multiple oral meds that did not seem to help. Even sleeping was painful, and they had told me that that ice helped better than the medication. The patient’s provider made it clear to the patient that a topical treatment would not work given the severity of zoster lesions. I thought, maybe a compounded preparation just might. I worked with the prescriber and compounded a topical combination cream that resolved most of the pain in two or three days, and the patient was sleeping again with no problem. By day seven, the lesions were resolving and dried up, and by day 10, everything was clear. The only remaining visible sign of zoster was some red skin where the worst of the shingles had been.

What has been your most satisfying patient experience? What was their health challenge? How did you assist them?

This is another question where there are many possible answers. Yet I believe working with HIV-infected children was one of my most satisfying professional experiences. Pediatric antiretroviral medication was so new at the time, and these kids were taking many medications multiple times a day. It was difficult for them to adhere to medications and the schedule. To help support adherence, we did many flavored medications and dosage form conversions. Groups of children with HIV would make visits to the lab to watch us make their medications, and we even gave them the opportunity to pick their own flavor. Our goal was to give them a sense of autonomy and control over their illness and medication. Many of these children are now young adults and likely have children of their own, but I still remember their faces and names.

What was the moment you realized that joining PCCA was going to make a difference for your pharmacy?

Immediately, during a PCCA training in Houston back in 1994. And PCCA keeps making a difference for my pharmacy to this day.

What is your favorite PCCA base?

Currently, NataTroche^®. My favorite bases change depending on what PCCA comes out with, but right now, it is NataTroche. It is easy to work with, the patients like it and we are currently evaluating everything we make in a troche mold to see if we can switch them to this base.

What is your favorite PCCA event?

International Seminar, of course. It brings everyone together for a great networking event. And there are so many educational opportunities during this event, from CE to lectures and speakers and an informative exhibit hall.

When was the last time a patient thanked you, and why?

Last week and the week before. We get a lot of thank-yous, some in the form of emails, letters or phone calls. We even get food and drink! Most of these gestures are a way for patients to express how good of a job we did in helping them or that what we compounded for them worked better than anything else they have tried.

What do you wish you could tell new compounders?

Network, network, network. Surround yourself with people who know more than you. And never say no to a patient’s need for a solution. Always, always try to find the answer.

What is your favorite thing about your job right now?

Building and growing my practice by learning new things, solving new problems, and watching my employees grow into great compounders.

What is your personal motto or words to live by?

When one grows, we all grow.

What do you enjoy for fun?

Spending time with my wife and daughter, martial arts, car restoration and car shows, and the gym.

I had heard about PCCA, so when I went out on my own, I knew what they offered would be helpful to me

What has been your most satisfying patient experience?

I had an adult female patient who had not been able to wear a pair of jeans for 10 years due to vaginal pain. We worked with her prescriber, who decided on a compound with diazepam in it. The next week, she came in crying and thanking me because she was so happy.

What’s the biggest “aha” moment you’ve had as a member of PCCA?

PCCA taught me how to help patients and keep them for life.

What’s your favorite PCCA base, and why?

PracaSil^®-Plus. It makes the skin feel smooth and is easy to work with.

What’s your favorite PCCA educational event, and why?

International Seminar. I am attending virtually this year. I also enjoyed C4 HRT online training and the subsequent HRT symposium.

Did you always want to work in the field of pharmacy?

I accidentally fell into pharmacy life. I was a line cook, barely able to buy diapers, and a friend told me to come tour a trade school with them because they didn't want to go alone. That friend didn't like the amount of work involved, but I loved everything I saw. I registered the following week and started working at a retail pharmacy. Before I knew it, I was falling more in love with the field of pharmacy at each place I was lucky enough to work.

When did you learn about compounding? Why did you start compounding?

I was working at a retail pharmacy with an intern who happened to come to us after finishing his rotation at Stone Oak Pharmacy. The way he talked about how the specialty pharmacy made medications intrigued me to the point that I had to look into it more. I enjoyed being a tech, but being a compounding technician seemed like a path I wanted to go on.

What do you find most fulfilling about compounding?

Compounding, for me, is very specific — the details, the steps, the learning. Everything makes a difference, and being able to compound medications makes me happy.

What are some of your favorite PCCA formulas? What do you like about them?

PCCA Formula #11432 (rectal rocket suppositories with lidocaine, hydrocortisone and tranexamic acid) is my favorite to make. Yes, the mold can be time consuming and takes some practice. But the challenge of making this formula is so much fun for me. The same can be said for PCCA Formula #6868 (oil-filled capsules). It isn't the standard formula. It isn't something that can be done quickly. These are formulas that make me want to learn more. The hard ones are the best ones.

What is a memorable story from your time in compounding?

This year has impacted everyone. In San Antonio, we not only struggled as a pharmacy but as a community. With the limitations on personal protective equipment (PPE), tests and personnel, the hospitals sought assistance from local compounders. We used PCCA formulas to meet those demands. Finding the formulas and learning the hows, whys and standards for them were not only fun and new for me, but we were also able to provide our community with much-needed preparations for their own protection. Supplying these preparations for our community was one of the best things I could be proud to be a part of. We were standing strong together and helping our health care family stay safe.

What advice would you give to a new compounder?

There is always something new to learn. Do not expect to know everything all at once. Take your time to not only understand the concepts and procedures, but also to be able to explain them to others. It makes everything easier for everyone.

How did you start compounding?

There was a genuine need in our community because there were no other compounding pharmacies in the immediate area. Since we already had a compounding lab in one of our other locations, we felt it just a continuation to go ahead and build a compounding lab in our pharmacy here. Then we made a business decision to build a second compounding lab. More and more, people are taking charge of their health, and that was becoming evident with the amount of compounding prescription requests we were receiving from prescribers.

What led you to PCCA?

Over time, we realized all the important aspects of being able to run and manage a successful compounding lab came from PCCA. Everything they did was cutting edge. From technical support to ordering support to event planning to PK Software, the team at PCCA is professional, and the support is second to none. In order to operate successfully, we needed the best support team providing the quality raw ingredients to provide our patients with the very best choices!

What has been your most satisfying patient experience?

This one happened very recently. We have been working with a patient who has had three unsuccessful attempts at in vitro fertilization. The fourth one worked, and her physician’s protocol included compounded progesterone pessaries, which we were able to help with. Recently, she brought her baby into the pharmacy. We were so genuinely happy for the family and proud to be part of their journey. She said thank you to our whole team.

What’s the biggest “aha” moment you’ve had as a member of PCCA?

Learning how all the calculations actually work — that moment that you get it!

What is your favorite PCCA base, and why?

LoxOral^® gives an option for patients with sensitivities. It reduces static and cuts capsule-packing time down by at least half, we would estimate. We also love SuspendIt^® with its unique thixotropic flow. We love ZoSil^® for animals as well.

When was the last time a patient thanked you, and why?

We get thanked every day from the patients who need compounds urgently, from the moms who haven’t slept because they have screaming babies needing omeprazole, to the local vets for whom we are able to provide fast, efficient service, to the patients who have let themselves run out of their medication, and when we say, “OK, well you can pick it up this afternoon,” and they say, “Oh thank you.”

Take it one at a time; look after each and every patient; and in time, the business will build.

How did you start compounding?

MedQuest Pharmacy started compounding in 1996 to meet a personal need for high quality and consistent personalized medication. PCCA has been with us since the beginning, offering training, formulas, consulting services and high-quality active pharmaceutical ingredients.

What has been your toughest patient problem?

When MedQuest started, the first few prescribers we worked with had patients from all over the United States. They would travel to meet these prescribers for their individual health needs and then would need ongoing help, education, services and medications thereafter. This created several problems and opportunities. We saw a need to facilitate the process between prescriber, patient and testing lab companies. This led to us creating MedQuest Testing Services, which helped patients and prescribers get the information they needed to develop their treatment protocols.

What has been your most satisfying patient experience?

I get to see all of the patient feedback we receive, and it is very rewarding to know how many patients sincerely appreciate the services and benefits we provide. It is also fun to share the patient feedback with the pharmacy team, as they enjoy working with our patients and prescribers. It is equally rewarding to know they make a difference in our patients’ quests to live better lives.

What’s the biggest “aha” moment you’ve had as a member of PCCA?

Getting involved in advocacy has been my biggest personal “aha” moment as a member of PCCA. One of the biggest benefits that PCCA offers is their Public Affairs team. There are considerable amounts of time, effort and resources that PCCA invests for the benefit of the entire industry, patients and prescribers. Their advocacy efforts on building awareness about compounding and the importance of preserving patient access to compounded medications makes a significant difference. From my first PCCA ACT Legislative Conference in Washington, D.C., I could see the benefits of getting more involved. There are great resources and information on the Public Affairs webpage, and their team is always willing to help out. I would highly encourage every PCCA member to attend PCCA ACT, become familiar with the Public Affairs resources available, be introduced to the talented Public Affairs team, and help us take advocacy and awareness even further. Having had several members of Congress visit MedQuest Pharmacy, I would also highly encourage reaching out to your elected officials for tours and visits of your own pharmacies.

What is your favorite PCCA base, and why?

VersaBase® Cream. It’s hypoallergenic and offers great absorption. It’s easy to work with. It also has great FormulaPlus™ data that extends beyond-use dates, which is extremely beneficial.

What is your favorite PCCA educational event, and why?

PCCA ACT is definitely my favorite educational event, and I would encourage all PCCA members to attend. It is a great opportunity to meet with lawmakers about pharmaceutical compounding and explain how regulations are affecting your patients. The importance of advocacy and awareness should be emphasized more, and I would encourage everyone to get involved.

When was the last time a patient thanked you, and why?

Since I receive all patient feedback, we receive thanks every day. Sometimes it is the time spent with a patient in counseling. Often it is a compliment on a pharmacy technician’s attention to detail. Sometimes it is based on the impact that we have had on a patient’s health and life. I like to share the positive feedback with the team and reward those individuals mentioned.

What advice would you give to a new compounder?

Attend as many educational and training opportunities as you can. Learn from others and ask for help. Get involved in advocacy and make it a part of your practice. Invite your representatives from the House and the Senate to visit your pharmacy, and help them learn more about compounding.

What is your favorite thing about your job right now?

My favorite thing about my job is definitely the talented people I get to work with, support and serve. The team is the reason we are successful. I also enjoy having our elected officials visit, tour and become champions of compounding.

What is your personal motto or words to live by?

To quote Steve Prefontaine: “To give anything less than your best is to sacrifice the gift.”

How did you start compounding? What led you to PCCA?
My father, Gary, and I started compounding when we joined PCCA. At the time, we had one pharmacy, the Clinic Pharmacy in Melrose, Minnesota. We were introduced to PCCA by a good friend and pharmacy classmate who had begun working at a compounding pharmacy in Phoenix, Arizona. After visiting that pharmacy and compounding lab, I knew this was the direction I wanted to proceed.

What was your toughest patient problem? How did you solve it?
We have many challenges with patients who need bioidentical hormone replacement therapy (BHRT), as many pharmacists do. Daily, those are the greatest challenges, and the most rewarding. We work closely with a few BHRT practitioners, and together we dig our heels in and try to gather enough pieces of the puzzle to make the picture clearer and help the patients move toward a more balanced state of health.

What has been your most satisfying patient experience?
No one situation stands out above the rest. Thankfully, between working with patients who need BHRT mainly and those who are prescribed low-dose naltrexone (LDN), we are in the position to experience patient successes every day. We are excited about the opportunities that LDN compounds are bringing to our pharmacy and patients. There are a number of practitioners in our area who have been prescribing various protocols of LDN to address a range of autoimmunity/inflammatory, pain and mental health conditions patients have been diagnosed with. We have also seen more and more benefits through our pharmacists taking on a greater consultative role in the process with LDN patients, much like our BHRT clients.

What is your favorite PCCA base, and why?
PCCA has many incredible proprietary bases that we utilize, and we absolutely believe they enhance patient outcomes. Many of our clients have experienced very positive results using PracaSil®-Plus, XemaTop™ and Clarifying™ bases alone as cosmetic products, with no active ingredients added. We have many clients who have had particularly fantastic outcomes with PracaSil-Plus as a cosmetic to improve the appearance of both newer and long-term scar tissue.

What is your favorite PCCA event?

International Seminar. I always enjoy the fellowship, and the educational components are superb. Every year, I come home with at least one great idea—typically more—that can be implemented that next week.

What advice would you give to new compounders?
Follow your instincts to help and serve your patients. There are a few obstacles and distractions that you will face along the way; don’t allow those to interfere with your passion of helping your patients achieve positive outcomes.

Do you have a motto or words you live by?
Be fearless in the pursuit of what sets your soul on fire.

What word best describes your life?
Rewarding, especially when it comes to spending time with my wife, Lisa, and our four children, Nolan, Lauren, Neal and Sophia.

A PCCA member since 2016, Megan attained her Bachelor of Science in Biology from East Carolina University; MBA from Lundy-Fetterman School of Business at Campbell University; PharmD from Campbell University School of Pharmacy; and a Fellowship in Anti-Aging, Metabolic and Functional Medicine through A4M.

How did you start compounding? What led you to PCCA?
During pharmacy school, I was fortunate enough to attend a PCCA Pharmacy Student Boot Camp in Houston, and I just fell in love with the art of compounding. After 12 years of filling up my patients' grocery-size bags with medications in retail pharmacy, I decided that I’d had enough abuse from the large chain pharmacy where I was working and I reached out to PCCA. I got inspired to go back into compounding!

What was your toughest patient problem? How did you solve it?
Usually, the tough patients are the ones with high sensitivity to chemicals and bases. In my experience, these patients have been overexposed to either environmental toxins or chronic mold. Helping them find a topical base can be very challenging. I often will give them samples of different bases to try prior to adding active ingredients. Once, I ended up using emu oil as a vehicle.

What has been your most satisfying patient experience?
I often identify therapy goals with my BHRT patients prior to starting them on their hormones or wellness program. The most satisfying patient experience is when the patient meets or exceeds their goal — when I am a part of their journey to better health and they finally see a difference in their quality of life. Whether it be a mom getting her energy back to take care of her family or a 20-year-old college student who went from not being able to get out of bed to now traveling the world, their stories are constant reminders for me to do better every day.

What’s the biggest “aha” moment you’ve had as a member of PCCA?
The answers to how to keep growing your business are through continued learning and staying on the cutting edge of information. My biggest "aha" moment was after I received my C4 HRT specialist designation with PCCA. I realized that, in addition to looking inside my organization for opportunities to grow, I needed to also look beyond what I already know and get more education. Through new information, networking and ideas that I got from the HRT program, I implemented what I learned in my practice and was able to keep my business on growing steadily.

What is your favorite PCCA base and why?
Atrevis Hydrogel® because my patients are getting incredible results from absorption and I'm getting great feedback from the doctors.

What is your favorite PCCA educational event and why?
International Seminars, because they’re fun and provide great opportunities to meet amazing people.

How did you start compounding? What led you to PCCA?

We believed in helping our community and that PCCA was a company that provides high quality chemicals and education, and that made them the best choice help us help our community. There were over ten pharmacies in our city, only one of which was a compounder. We decided to jump in knowing PCCA would allow us to have access to the highest standard of product, which in turn, gave us the ability to provide the highest quality to our patients.

What was your toughest patient problem? How did you solve it?

We’ve had quite a few over the years. Recently, we had a young patient who has extremely sensitive skin and fair skin. She needed a sunscreen and had pretty much tried everything. After heaps of trial and error, we used VersaBase cream and found a combination that finally worked. She had no reaction and loved the product!*

What has been your most satisfying patient experience?

We had a patient who came into the pharmacy early on and had been having so much trouble with her hormone replacement therapy and getting her hormone levels to stabilize. She had been getting troches from another compounder, but didn’t like the product, and her levels weren’t getting where her physician wanted them. She attempted several trials with multiple compounders, but found no success. After nearly a year’s worth of tests, we figured out the right combination and found that she loved the peppermint flavor because it masked the bitter active ingredients! She’s been coming back ever since.

What is your favorite PCCA base, and why?

VersaBase®. It is such a versatile base — it speaks for itself. We use the cream version because we’ve found it’s fantastic with both compatibility and has the best feel of all the creams we use.

What is your favorite PCCA educational event?

HRT seminar. There is so much new information, and it’s a great opportunity to see and hear what is up and coming.

When was the last time a patient thanked you?

We had a young patient with severe reflux, a desperate and tired mother, and thankfully a prescription for a compound with omeprazole. The mother called back a week later thanking us for making it for her child as soon as possible.

What advice would you give to new compounders?

Keep asking questions, and keep double-checking information. There’s a world of information out there, and the PCCA teams is always happy to help.

What is your favorite thing about your jobs right now?

Since our 2018 Australian Guild Pharmacy of the Year win, we’ve had the opportunity meet some fantastic people in the industry. Sharing ideas and collaborating with these people has definitely been a recent highlight.

What is your personal motto or words you live by?

Luke: I channel advice from my hero, David Brent [from BBC’s The Office]: “People say to me, ‘Would you rather be thought of as a funny man or a great boss?’ My answer's always the same: ‘To me, they're not mutually exclusive.’"
